Position Overview:

We are seeking a Community Support Worker for a full-time permanent role to enhance our mission of providing comprehensive support.

Role Summary:

The Community Support Worker will play a crucial role in establishing a network of support for individuals living with mental health conditions, either in a program residence or independently within the community. This position involves delivering a variety of assistance and support services aimed at fostering independence, dignity, and self-respect among clients. The worker will be part of a dedicated team that coordinates resources for adults facing intricate mental health, health, and addiction challenges.

Key Responsibilities:
  • Conducting client intake assessments to identify daily living needs and collaboratively developing personalized support and recovery plans.
  • Building rapport with clients to support their individual journeys, assisting with daily living activities, and promoting life skills such as hygiene, medication management, and budgeting.
  • Creating a holistic support network by connecting clients with social, family, and community resources, ensuring ongoing follow-up and support.
  • Advocating for clients within social services and healthcare systems to ensure they receive necessary services.
  • Encouraging participation in group activities and meetings that enhance resident wellbeing and community engagement.
  • Maintaining accurate records and documentation related to client activities and program administration.
Qualifications:

Essential:

  • A solid understanding of harm reduction and psychosocial rehabilitation principles, supported by a relevant degree or diploma in human services.
  • A minimum of three years of experience working with similar client populations.
  • Familiarity with the challenges faced by individuals with mental health and addiction issues.
  • Ability to establish therapeutic relationships and negotiate goals with clients.
  • Knowledge of local community resources and services available to clients.
  • Experience collaborating within a professional team and facilitating group discussions.
  • Strong interpersonal, communication, and counseling skills.
  • Proficient in crisis intervention, advocacy, and problem-solving.
  • Competent in using technology for record-keeping and data management.
  • Excellent organizational and time management skills.
