Position Overview:
LOFT Community Services is a distinguished charitable organization dedicated to assisting individuals facing complex challenges, including mental health issues, substance use, poverty, and homelessness. Our mission is to provide comprehensive support to enhance the lives of those we serve.
We are currently seeking a Community Support Worker to join our team on a full-time permanent basis. This role is pivotal in establishing a robust support network for individuals living with mental health challenges, whether in a residential setting or within the community.
Key Responsibilities:- Engage in the intake process for clients, conducting assessments to identify their daily living needs and collaboratively developing personalized support and recovery plans.
- Foster supportive relationships with clients, assisting them in achieving their daily living goals, particularly in acquiring essential life skills such as hygiene, medication management, cooking, and budgeting.
- Build and maintain a holistic support network that includes social, family, and community resources, ensuring clients have access to necessary services through follow-ups and accompaniment to appointments.
- Advocate for clients within social services and healthcare systems, ensuring they receive the support and accessibility they require.
- Encourage client participation in house meetings and community activities that promote well-being and enhance the operational efficiency of the residence.
- Assist in program administration by maintaining accurate records and statistics related to resident files and activities.
Essential:
- A solid understanding of harm reduction and psychosocial rehabilitation principles, supported by a relevant degree or diploma in human services.
- A minimum of three years of experience working with individuals facing mental health and addiction challenges.
- Familiarity with the issues encountered by adults experiencing mental health challenges or homelessness.
- Ability to establish therapeutic relationships and negotiate goals with clients effectively.
- Knowledge of local community resources and services available to the client population.
- Experience working collaboratively within a professional team and facilitating group meetings.
- Strong interpersonal, communication, and counseling skills.
- Proficient in crisis intervention, advocacy, and problem-solving.
- Competent in computer usage for maintaining records and statistical information.
- Excellent organizational and time management skills.
- Willingness to be on-call during after-hours on a rotating basis.
- Ability to travel in various weather conditions and navigate multiple community locations.
