Position Overview:

LOFT Community Services is a prominent charitable organization dedicated to assisting individuals facing complex challenges, including mental health issues, substance use, poverty, and homelessness. We are seeking a Community Support Worker to join our dedicated team on a full-time, permanent basis.

Role Summary:

The primary objective of this role is to establish a robust support network for individuals living with mental health conditions, whether in a residential program or independently within the community. As a Community Support Worker, you will collaborate with a team to coordinate essential services for adults experiencing intricate mental health, health, and addiction challenges. A significant aspect of this position involves fostering a network of community resources that empower individuals to enhance their independence, dignity, and self-worth.

Key Responsibilities:
  • Engage in the intake process for clients referred to the program, conducting assessments to identify their daily living needs and collaboratively developing support and recovery plans.
  • Support each individual's personal journey by building trusting relationships and assisting them in achieving their daily living requirements, focusing on life skills such as hygiene, medication management, cooking, cleaning, shopping, and budgeting.
  • Create a comprehensive support network that includes social, family, spiritual, and community resources, ensuring ongoing follow-up and support through reminders and accompaniment to appointments.
  • Advocate for clients within social services and healthcare systems, ensuring they have access to necessary services and support.
  • Encourage participation in house meetings and community activities that promote resident wellbeing and the effective operation of the program.
  • Assist in program administration by maintaining accurate records and statistics, including resident files and activity reports.
Qualifications:

Essential:

  • A solid understanding of harm reduction and psychosocial rehabilitation principles, supported by a relevant degree or diploma in human services.
  • A minimum of three years of experience working with similar client populations.
  • Familiarity with the challenges faced by adults dealing with mental health and/or addiction issues.
  • Ability to establish therapeutic relationships and negotiate goals with clients.
  • Knowledge of local community agencies and resources available to the client group.
  • Experience working collaboratively within a professional team and facilitating group meetings.
  • Strong interpersonal, communication, and counseling skills.
  • Proficient in intake assessment, crisis intervention, advocacy, and problem-solving.
  • Computer literacy for maintaining files and records.
  • Effective analytical, time management, and organizational skills.
  • Willingness to be on-call after hours on a rotating basis.
  • Ability to work in various environments, including resident rooms, and manage exposure to secondhand smoke.
  • Capability to travel in all weather conditions and navigate multiple community locations daily.

  • Awareness of the issues faced by individuals identifying as 2SLGBTQ+.
