Salary: External Rate – E7: $42,843.24
Internal Rate – E8: $43,717.59
Program Overview – Supportive Housing, Community Care
The Supportive Housing Program is dedicated to providing accommodation for seniors who have faced homelessness and may be dealing with mental health challenges or substance use issues. This program offers comprehensive onsite psychosocial and primary care support services, including intensive case management, harm reduction strategies, therapeutic counseling, 24-hour staffing, assistance with daily living activities, nursing care, life skills training, social and recreational activities, volunteer opportunities, and ongoing support.
Key Responsibilities
  • Engage with program participants in person, offering emotional support and encouragement to enhance their involvement in the program.
  • Deliver individualized support as specified in Coordinated Care plans, assisting social workers with case management tasks such as form completion and navigating formal systems.
  • Facilitate the intake process, including applicant selection, interviews, and follow-up with new participants.
  • Monitor the progress and well-being of participants, assisting the team in assessing the effectiveness of Coordinated Care Plans and suggesting modifications as needed.
  • Contribute to the goal-setting and Coordinated Care planning processes.
  • Design, develop, and implement relevant individual and group programming for participants.
  • Assist participants in achieving greater independence within the program and the community.
  • Regularly utilize the ADL/IADL assessment tool.
  • Participate in housing stabilization planning for participants, including application support, waitlist monitoring, and collaboration with housing assistance organizations.
  • Engage in skill development activities to help individuals perform daily living tasks such as grooming, hygiene, shopping, banking, and informal safety training.
  • Support individuals in activities that promote their independence and community engagement, identifying services and activities that meet their needs while providing guidance and support.
  • Collaborate with other service providers and the community to build and strengthen networks.
  • Recognize and assess potential aggressive behavior or crisis situations, developing strategies to address these incidents and informing the team and management as necessary.
  • Execute delegated tasks as directed by healthcare professionals, including medication management in accordance with primary care instructions.
  • Coordinate transportation for participants and may accompany them to appointments and community activities as needed.
  • Efficiently complete and maintain documentation and case notes.
  • Stay informed about internal safety regulations and emergency procedures, ensuring compliance with health and safety standards.
  • Contribute to program development and participate in program evaluations.
Qualifications
  • Social Service Worker diploma or related educational background.
  • Minimum of 2 years of experience working with older adults facing mental health and substance use challenges.
Preferred Skills
  • Ability to effectively engage with high-risk, marginalized, and vulnerable populations.
  • Experience managing aggressive behaviors, with strong de-escalation and boundary-setting skills.
  • Excellent written, verbal, and interpersonal communication abilities.
  • Capacity to work independently as well as collaboratively within a team.
  • Proficiency in conflict resolution and crisis intervention techniques.
  • Demonstrated sound judgment.
  • Competence in using computers, databases, and web-based applications.
  • Knowledge of geriatric issues, harm reduction principles, and mental health legislation.
  • Familiarity with community resources and various systems.
