Position Overview:

LOFT Community Services is a prominent charitable organization dedicated to assisting individuals facing complex challenges, including mental health issues, substance use, poverty, and homelessness. We are seeking a Community Support Worker to join our team on a full-time permanent basis.

Role Summary:

The primary responsibility of the Community Support Worker is to establish a support network for individuals experiencing mental health challenges, whether they reside in a program facility or independently within the community. This role involves providing comprehensive assistance and support to clients, ensuring they have access to necessary community resources. You will collaborate with a team to coordinate support for adults with intricate mental health, health, and addiction challenges within our Layered Supports Program.

Key Responsibilities:
  • Conducting client intake assessments to identify daily living needs and collaboratively developing support and recovery plans.
  • Building rapport with clients and supporting their personal journeys by assisting with daily living activities, including hygiene, medication management, cooking, cleaning, shopping, and budgeting.
  • Creating a holistic support network by linking clients to social, family, community medical, and psychiatric resources, while providing follow-up and accompanying clients to appointments as needed.
  • Advocating for clients within social services and healthcare systems to ensure they receive accessible and available services.
  • Encouraging client participation in house meetings and community activities that promote well-being and the effective operation of the residence.
  • Maintaining accurate records and statistics related to resident files, income-based rent files, and activity reports.
Qualifications:

Essential:

  • A solid understanding of harm reduction and psychosocial rehabilitation principles, supported by a relevant degree or diploma in human services.
  • A minimum of three years of experience working with similar client populations.
  • Familiarity with the challenges faced by adults experiencing mental health issues, addiction, or homelessness.
  • Ability to foster therapeutic relationships and negotiate goals with clients.
  • Knowledge of local community resources and services available to the client demographic.
  • Experience working collaboratively within a professional team and facilitating group meetings.
  • Strong interpersonal, communication, and counseling skills.
  • Proficient in crisis intervention, advocacy, and problem-solving.
  • Computer literacy for maintaining files and records.
  • Effective analytical, time management, and organizational skills.
  • Willingness to be on-call after hours on a rotating basis.
  • Ability to travel in various weather conditions and navigate multiple community locations.

  • Understanding of the issues faced by individuals identifying as 2SLGBTQ+.
