We are seeking a skilled Patient Navigator to join our team at Vision Loss Rehabilitation Canada. This is an exciting opportunity to make a difference in the lives of individuals with vision loss.

In this role, you will work closely with our clients, their families, and healthcare professionals to provide patient-centered care and support. Your primary responsibility will be to coordinate and integrate supports and services around the individual, ensuring they have timely access to necessary resources and information.

The ideal candidate will possess excellent communication and interpersonal skills, as well as experience working in a health or social service setting. A diploma in Social Services Work, Community Work, or Mental Health would be considered an asset. Additionally, having worked with individuals with vision loss and possessing cultural sensitivity and diversity skills would be beneficial.

The successful candidate will be responsible for:

  • Scheduling appointments with primary and specialized service providers
  • Monitoring client satisfaction and taking remedial action as needed
  • Providing proactive follow-up to ensure ongoing collaboration across services and providers
  • Identifying opportunities for new or emerging care options to meet changing needs and address service gaps
