Welcome to Vision Loss Rehabilitation Canada, a unique health services organization that empowers individuals with vision loss to develop or restore activities of daily living skills and promote their independence.

About the Role

We are seeking a highly skilled Client Care Navigator to coordinate and integrate supports and services around individuals with vision loss. As the key contact or navigator for clients after a vision loss diagnosis, you will assess their needs, support them in achieving their care plan goals, and strengthen communication and linkages between VLR staff and external community healthcare providers across the continuum of care.

Responsibilities
  • Schedule all VLR therapy appointments with primary service providers and specialized service providers.
  • Seek feedback on client satisfaction and take remedial action as required.
  • Monitor and provide proactive follow-up for clients to ensure ongoing collaboration across services/providers.
  • Identify opportunities for new or emerging care options to meet changing needs and address service/support gaps.
  • Educate community partners about Vision Loss Rehabilitation Canada and its specialized services offered to people with vision loss.
Requirements

To be successful in this role, you will require:

  • An understanding of roles and linkages across primary care, community care, and specialized geriatric services.
  • Clinical expertise related to client-centered interviewing, assessments, and scheduling practices.
  • Experience in client assessments and care planning/coordination.
  • Excellent communication skills (verbal and written).
  • Strong interpersonal skills, including shared decision-making and facilitation.
