Job Overview

We are seeking an experienced Healthcare Navigator to join our team in Kingston or Ottawa, ON. As a key member of our team, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.

Key Responsibilities:
  • Coordinate and integrate supports and services around individuals living with vision loss
  • Assess client needs and develop personalized care plans
  • Strengthen communication and linkages between VLR staff and external community healthcare providers
  • Provide timely access to VLR service information and connection to the healthcare system
Requirements
  • Experience working in the health and social service sectors
  • Diploma in Social Services Work, Community Work, Mental Health, or related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and within a team
  • Proficient in technology, including Microsoft Office and case management systems
Salary and Benefits

The successful candidate will receive a competitive salary of $27.71/hour, plus benefits and opportunities for professional development and growth.
