Position Overview

The Vision Support Coordinator will play a crucial role in orchestrating and unifying the services and assistance provided to individuals experiencing vision impairment. Acting as the primary liaison for clients following a vision loss diagnosis, the Coordinator will focus on assessment, goal achievement, and enhancing communication between Vision Loss Rehabilitation Canada (VLR) staff and external healthcare providers throughout the care continuum.

Key Responsibilities

This role encompasses a variety of essential functions, including but not limited to:

  • Coordinating all therapy appointments with both primary and specialized service providers.
  • Gathering client feedback to assess satisfaction and implementing necessary improvements.
  • Providing proactive follow-up to ensure ongoing collaboration among various service providers.
  • Identifying and proposing new care options to address evolving client needs and service gaps.
  • Documenting all service-related activities in accordance with VLR policies.
  • Engaging with clients, their families, and caregivers to establish vision-related objectives.
  • Fostering strong relationships with professionals across different agencies and community organizations.
  • Educating community partners about the specialized services offered by Vision Loss Rehabilitation Canada.
  • Participating in interdisciplinary team meetings to enhance service delivery.
  • Facilitating effective communication and coordination between clients and healthcare service providers.
  • Understanding the roles and connections between primary and specialized service providers within the service delivery framework.
  • Maintaining partnerships through knowledge sharing while adhering to confidentiality protocols.
  • Managing caseloads and maintaining accurate electronic medical records.

Qualifications

Knowledge and Skills

  • 3 to 5 years of experience in client services within health or social service sectors.
  • Experience working directly with individuals facing vision loss.
  • Knowledge in managing chronic and complex health conditions.
  • Familiarity with community services and various care options.
  • Understanding of the interplay between primary care, community care, and specialized services.
  • Strong client-centered approach.
  • Proficient in client assessments, care planning, and coordination.
  • Excellent verbal and written communication skills.
  • Exceptional interpersonal skills, including shared decision-making.
  • Ability to prioritize tasks and manage competing demands.
  • Resourceful with strong problem-solving capabilities.
  • Proficient in technology, including Microsoft Office and case management systems.
  • Ability to work independently and collaboratively within a team.
  • Experience in cultural sensitivity and diversity.

Education and Experience

  • Registered health professional designation is an asset.
  • Bachelor's degree in Social Work, Gerontology, or a related healthcare field.
  • Minimum of 3 years of experience working with individuals with vision loss.
