Position Overview
The Vision Support Coordinator will play a pivotal role in orchestrating and harmonizing the array of services and assistance available to individuals experiencing vision impairment. This direct service position will serve as the primary liaison for clients following a diagnosis of vision loss. Key responsibilities encompass evaluating client needs, aiding in the realization of care plan objectives, and enhancing communication and collaboration between Vision Loss Rehabilitation Canada (VLR) staff and external healthcare providers throughout the care continuum.
Key Responsibilities
The following are essential functions of this role:
- Arrange all therapy appointments with both primary and specialized service providers.
- Gather client feedback to assess satisfaction and implement necessary improvements.
- Conduct proactive follow-ups to ensure continuous collaboration among service providers.
- Identify and propose new care options to address evolving client needs and service gaps.
- Document all service-related activities in the client health record in accordance with VLR policies.
- Engage with clients, their families, and caregivers to establish and pursue vision-related goals.
- Foster and maintain positive relationships with professionals from other agencies and community organizations.
- Educate community partners about the services offered by Vision Loss Rehabilitation Canada.
- Participate in interdisciplinary team meetings to enhance service delivery.
- Facilitate effective communication and coordination between clients and healthcare service providers.
- Understand the roles and interactions between primary and specialized service providers within the service delivery model.
- Build partnerships through knowledge sharing while ensuring confidentiality in accordance with VLR policies.
- Manage caseloads effectively, ensuring accurate medical documentation and maintenance of electronic health records.
Qualifications
Knowledge and Skills
- 3 to 5 years of experience in client service within health or social services.
- Experience working directly with individuals facing vision loss.
- Knowledge in managing chronic and complex health conditions.
- Familiarity with community services and various care options.
- Understanding of the connections between primary care, community care, and specialized services.
- Strong client-centered approach.
- Proficient in client assessments, care planning, and coordination.
- Excellent verbal and written communication skills.
- Exceptional interpersonal skills, including facilitation and shared decision-making.
- Ability to prioritize tasks and manage competing demands.
- Resourceful with strong problem-solving capabilities.
- Proficient in technology, including Microsoft Office and case management systems.
- Ability to work independently and collaboratively within a team.
- Experience in cultural sensitivity and diversity.
Education and Experience
- Registered health professional designation is an asset.
- Bachelor's degree in Social Work, Gerontology, or a related health discipline.
- 3 or more years of experience working with individuals experiencing vision loss.
