Position Overview
The Vision Support Coordinator will play a pivotal role in orchestrating and unifying services and assistance for individuals experiencing vision impairment. In this direct service capacity, the Coordinator will serve as the primary liaison for clients following a vision loss diagnosis. Key responsibilities encompass evaluating client needs, aiding them in reaching their care objectives, and enhancing communication and connections between Vision Loss Rehabilitation Canada (VLR) staff and external healthcare providers throughout the care continuum.
Core Responsibilities
The essential functions of this role include, but are not limited to:
- Coordinating all therapy appointments with both primary and specialized service providers.
- Gathering feedback on client satisfaction and implementing necessary improvements.
- Providing proactive follow-up to ensure continuous collaboration among service providers.
- Identifying new care options to address evolving needs and service gaps.
- Documenting all service-related activities in client health records in accordance with VLR policies.
- Engaging with clients, their families, and caregivers to establish vision-related goals.
- Fostering strong working relationships with professionals from other agencies and community organizations.
- Educating community partners about the specialized services offered by Vision Loss Rehabilitation Canada.
- Participating in interdisciplinary team meetings.
- Facilitating communication and coordination between clients and healthcare service providers.
- Understanding the roles and connections between primary and specialized service providers within the service delivery model.
- Building partnerships through knowledge sharing while maintaining confidentiality as per VLR policies.
- Managing caseloads, medical documentation, and electronic health records.
Qualifications
Knowledge and Skills
- 3 to 5 years of experience in client services within health or social service sectors.
- Experience working directly with individuals facing vision loss.
- Knowledge in managing chronic and complex health conditions.
- Familiarity with community services, clinical, social, and residential care options.
- Understanding of the roles within primary care, community care, and specialized geriatric services.
- Strong grasp of client-centered care principles.
- Expertise in client assessments, care planning, and coordination.
- Proficient in inter-professional collaboration.
- Exceptional verbal and written communication skills.
- Strong interpersonal skills, including shared decision-making and facilitation.
- Able to prioritize tasks and manage competing demands effectively.
- Resourceful with strong problem-solving abilities.
- Proficient in technology, including Microsoft Office and case management systems.
- Ability to work independently and collaboratively within a team.
- Experience in cultural sensitivity and diversity.
Education and Experience
- Registered health professional designation is an asset.
- Bachelor's degree in Social Work, Gerontology, or a related healthcare discipline.
- 3 or more years of experience working with individuals with vision loss.
Work Environment
