Wage: PG10|$42.77-$53.44

Status: Temporary Full Time, 24 months

Dept. Health System: Quality and Practice

Hours of Work/Shifts: 7.5 Hour Days (Monday to Friday)

POSITION SUMMARY:

Trillium Health Partners (THP) is seeking a skilled Project Manager to contribute to the Model of Care (MOC) Initiative. This role will involve collaborating with the Clinical Project Director to facilitate the planning, execution, and expansion of the MOC project. The successful candidate will engage with key stakeholders to support various aspects of the initiative, including the creation of innovative tools and resources, as well as the documentation of critical work streams.

The Project Manager will utilize project management methodologies, data analysis, evidence-based practices, change management techniques, and improvement science to ensure the successful delivery of project objectives and timelines. The initial focus will be on the Inpatient Medicine Program, with plans for broader implementation across the organization's inpatient services.

This position requires effective collaboration with team members and leaders across multiple departments, including Foundations of Clinical Excellence, Quality, Risk, Practice, and others. The Project Manager will play a vital role in establishing the necessary conditions, processes, and tools that empower staff to achieve consistent clinical excellence and enhance overall wellness.

KEY RESPONSIBILITIES:

Oversee the development and execution of the model of care workplan, including communication strategies, milestone tracking, data collection, and reporting. Act as a liaison for the Foundations of Clinical Excellence and Senior Leadership Committee, providing updates and support materials related to the project. Monitor project progress, identify challenges, and propose solutions, ensuring effective communication with all stakeholders. Facilitate project work streams and committee meetings, offering guidance and motivation throughout the project lifecycle. Develop and implement change management strategies that foster employee engagement and ensure smooth transitions. Maintain ongoing communication with leaders and teams to address evolving resource and project needs. Contribute to the creation and delivery of impactful presentations and proposals for various audiences.

REQUIRED QUALIFICATIONS:

Bachelor's degree in business, life sciences, health informatics, or a related field. Master's degree in public administration, business administration, or healthcare administration is preferred. Relevant certifications in healthcare quality, project management, or change management are advantageous.

EXPERIENCE:

A minimum of 2 years of experience in project management or leading complex change initiatives within healthcare settings. Proven track record of facilitating cultural change in healthcare environments. Strong organizational, time management, and analytical skills. Proficiency in utilizing data to drive decision-making and results. Excellent communication skills, with the ability to build relationships and effectively engage with diverse teams. Demonstrated ability to manage multiple priorities and work effectively under pressure.
