Position Summary
Are you a Registered Occupational Therapist passionate about making a difference in people's lives? Northern Health is seeking a dedicated and skilled Occupational Therapist II to join our dynamic Primary Care team.
Within a person-centered care model, the Occupational Therapist II collaborates with interprofessional teams to develop comprehensive care plans and deliver exceptional health services through the Primary Care Home.
Key Responsibilities
- Conduct thorough assessments of patients' needs and functional abilities.
- Develop individualized treatment plans based on evidence-informed practice and patient goals.
- Provide therapeutic interventions to enhance patients' participation in meaningful activities.
- Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated care.
- Educate patients and families about their conditions and treatment options.
- Maintain accurate documentation of patient progress and interventions.
Qualifications
- Registered Occupational Therapist with current licensure in good standing.
- Demonstrated experience working in a primary care setting preferred.
- Strong clinical skills and knowledge of evidence-based practice.
- Excellent communication, interpersonal, and teamwork skills.
- Proficiency with electronic health records (EHR) systems.
