We are seeking a highly motivated and experienced Diabetes Care Specialist to join our dynamic Diabetes Clinic. In this role, you will play a vital part in delivering comprehensive diabetes management and education services to patients with diverse needs.

Responsibilities:

  • Provide expert clinical care to individuals with diabetes, including assessment, diagnosis, treatment planning, and ongoing monitoring.
  • Develop and implement individualized patient education plans tailored to specific diabetes types and complexities.
  • Collaborate effectively with a multidisciplinary team of healthcare professionals to ensure holistic patient care.
  • Stay abreast of the latest advancements in diabetes management and best practices.
  • Contribute to a positive and supportive work environment that fosters teamwork and professional growth.

Qualifications:

  • Registered member, in good standing, of the College of Dietitians of Ontario.
  • Certified Diabetes Educator preferred (candidates without CDE, will have limited scope related to insulin adjustment).
  • Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education with a focus in specialized and complex diabetes care, e.g., GDM, Pregnancy, Type 1 Diabetes, Insulin Pump Management.
  • Certified Insulin Pump Trainer preferred.
  • Experience in Paediatric Diabetes Management preferred.
  • Membership with Dietitians of Canada preferred.
  • 2 years' experience in complex nutrition support preferred.
  • Current teaching experience and counseling background.
  • Current experience with electronic documentation along with computer proficiency.
  • Current knowledge of normal and therapeutic nutrition, enteral feeds and TPN.
  • Bilingualism in French/English an asset.
