Job Summary

The University Health Network is seeking a highly skilled Patient Educator Diabetes Specialist to join our Diabetes/Endocrine Program team. As a key member of our multi-disciplinary team, you will play a vital role in providing patient education and support to individuals with diabetes.

Key Responsibilities

  • Develop and implement patient education programs to promote self-management of diabetes
  • Provide individualized education and support to patients with diabetes, including teaching medication self-administration and glucose monitoring
  • Collaborate with healthcare providers to ensure comprehensive care and support for patients with diabetes
  • Conduct health assessments and develop individualized care plans to meet the unique needs of patients with diabetes
  • Stay up-to-date with the latest research and best practices in diabetes care and education

Requirements

  • Current registration with the College of Nurses of Ontario
  • Basic Cardiac Life Support Certification
  • Excellent interpersonal, communication, and decision-making skills
  • Excellent organizational and time management skills
  • Experience working in a fast-paced ambulatory setting with patients with complex medical needs
  • Health assessment skills
  • Experience with patients with diabetes, lipid disorders, thyroid disease, osteoporosis, and disorders of other endocrine glands
