We are a leading health access centre providing holistic services to First Nation, Inuit and Métis people in Ottawa.

Salary: $60,000 - $80,000 per year

About the RoleCulture as Care Coordinator/Youth Team Lead

This role requires an experienced front-line youth worker to coordinate culture, case management and health promotion. The successful candidate will be responsible for guiding the youth team in their roles and meeting program targets and deadlines.

  • Liaise with CAS in northern communities and other organizations such as the Boys and Girls Club;
  • Build and maintain relationships with community partners such as CAS and group homes;
  • Coordinate and facilitate Indigenous integrated plans of care based on the model of care;
  • Advocate for social justice and policy change;
  • Provide case management and systems navigation to eligible youth and manage referrals from Indigenous and mainstream organizations.
Required Skills and Qualifications
  • Masters or Bachelors degree in Social Work or other applicable human service program;
  • 3-5 years of experience in Case Management;
  • Experience with youth programming and effective communication with Indigenous youth;
  • Knowledge of Indigenous people's history and challenges Indigenous youth face in contemporary society;
  • Demonstrated experience working with high-risk youth and managing crisis situations;
  • Knowledge of available resources in Ottawa and ability to research the resources available in remote communities outside of Ottawa;
  • Ability to work a flexible work week that may include non-traditional hours, evenings, and weekends.
