Care Team Manager

We are seeking a dynamic and compassionate individual to lead our youth team in their roles of culture, case management, and health promotion.

As a Culture as Care Coordinator/Youth Team Lead, you will be responsible for guiding the team in meeting program targets and deadlines while also assuming a hands-on role in supporting youth 16-24 to transition from care of CAS or formal customary care.

To meet these responsibilities, you will:

  • Liaise with CAS in northern communities, Ottawa, and other organizations such as the Boys and Girls Club, ROH;
  • Build and maintain relationships with community partners such as CAS and group homes;
  • Coordinate and facilitate Indigenous integrated plans of care based on the model of care;
  • Advocate for social justice and policy change;
  • Provide case management and systems navigation to eligible youth and manage referrals from Indigenous and mainstream organizations.

Qualifications

  • Masters or Bachelors degree in Social Work or other applicable human service program;
  • 3-5 years of experience in Case Management;
  • Experience with youth programming and effective communication with Indigenous youth;
  • Knowledge of Indigenous people's history and challenges Indigenous youth face in contemporary society;
  • Demonstrated experience working with high-risk youth and managing crisis situations;
  • Knowledge of available resources in Ottawa and ability to research the resources available in remote communities outside of Ottawa;
  • Ability to work a flexible work week that may include non-traditional hours, evenings, and weekends.

Salary and Benefits

We offer a competitive salary ranging from $60,000 to $80,000 per year, depending on experience. Comprehensive Group Health and Dental Insurance Coverage, and a Defined Benefit Pension Plan/ Employee Pension Plan with an equal contribution by Wabano (HOOPP).
