Job Summary

Mackenzie Health is seeking a skilled Nurse Practitioner to join our team as part of the Community Commitment Program for Nurses (CCPN). This is an exciting opportunity for a dedicated healthcare professional to provide high-quality patient-centered care and contribute to the advancement of nursing practice.

Key Responsibilities
  • Provide comprehensive health services to clients, including identification and management of chronic stable conditions, prevention of disease and injuries, health promotion and education, and rehabilitation.
  • Work collaboratively with clients and other healthcare providers to deliver high-quality patient-centered care.
  • Assess and provide services to clients, families, and communities, utilizing advanced nursing assessment skills and sound knowledge of clinical therapeutics.
  • Develop and implement education initiatives for healthcare providers and community members, demonstrating knowledge and experience of theories guiding adult learning principles and health promotion activities.
Requirements
  • Certificate of Registration (EC) in good standing from the College of Nurses of Ontario.
  • Commitment to two years of employment at a full-time work schedule offered by the employer.
  • Ability to work independently and as part of a team, with high-level initiative and self-direction.
  • Excellent organizational, critical appraisal, and analytical skills, with effective patient/family counseling skills and computer literacy in a Windows environment.
