Job Overview

This is a temporary full-time position for a duration of up to eighteen (18) months.

The Sinai Health System is seeking an Advanced Practice Nurse to join our dedicated team at the Vaughan site. In collaboration with a multidisciplinary team, the Advanced Practice Nurse will undertake clinical responsibilities within their professional scope, which includes patient assessments, diagnosis, treatment, follow-up care, counseling, screening, education, and referrals for all patients. This role will also involve the implementation of a program aimed at re-engaging patients who have missed essential preventative care due to recent global health challenges.

Clinical duties will be conducted through scheduled in-person and virtual consultations.

Key Responsibilities

Essential Qualifications:

Mandatory

  • Registered Nurse in the Extended Class with a specialization in Primary Health Care
  • Current registration in good standing with the regulatory body
  • Proven commitment to evidence-based practice and ongoing professional development

Preferred

  • Master’s degree in nursing
  • Eligibility for cross-appointment to an academic institution
  • Minimum of two years of clinical experience in primary care or a relevant outpatient care setting

Skills and Competencies

  • Ability to work autonomously and apply critical thinking in a diverse clinical setting
  • Knowledge or experience in preventative care and chronic disease management is advantageous
  • Strong clinical judgment along with excellent health assessment and diagnostic capabilities
  • Exceptional interpersonal skills and leadership qualities as an integral member of a multidisciplinary team
  • Active participation in knowledge sharing with students and other healthcare professionals
  • Experience in program development, evaluation, and change management
  • Capacity to assist in the formulation and review of clinical processes to ensure high-quality patient care
  • Commitment to equity, diversity, and inclusion principles within a community-focused context
  • Understanding of, and dedication to, patient and family-centered care
  • Demonstrated satisfactory work performance and attendance history
