About the Role

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at Pacific Coast Community Resources. As a Community Support Specialist, you will play a vital role in providing care and support to individuals living in staffed homes within their community.

Key Responsibilities
  1. Respect and Advocate: Ensure the supported persons' rights and choices are respected and advocate as needed.
  2. Effective Interactions: Interact effectively with people we support, fostering positive relationships and a sense of community.
  3. Empowerment and Independence: Empower the people we support to be full citizens of their community, promoting independence and self-determination.
  4. Personal Service Plans: Participate in the development, implementation, and documentation of Personal Service Plans, ensuring individualized care and support.
  5. Communication and Collaboration: Foster positive relationships with persons' family members, professionals, co-workers, and members of the community, promoting effective communication and collaboration.
  6. Record Keeping: Record the daily activity and personal health information (PHI) of the people we support on our case management database.
  7. Home Maintenance: Work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties.
Requirements
  1. Education and Experience: Community Support Worker Diploma or equivalent, with a minimum of 2 years of experience in community social services/health care.
  2. Certifications: Current Emergency First Aid/CPR and Food Safe certificates, previous behaviour management course/training, and a Restricted class 4 driver's licence (for Full-Time positions).
  3. Clearance and Vaccination: Criminal record clearance for children and vulnerable adults, and proof of COVID-19 vaccination status.
  4. Computer Literacy: Moderate computer literacy and ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
