Community Support Worker
**About the Role**
As a Community Support Worker with Pacific Coast Community Resources, you will be part of a dynamic team that provides care and support to individuals living in staffed homes within their community. Your role will be to empower and encourage people to be independent and self-determining, while maintaining excellent standards of care, health, and safety.
**Core Responsibilities**
* Ensure the rights and choices of supported individuals are respected and advocate as needed.
* Interact effectively with people we support, fostering positive relationships and promoting their full citizenship in the community.
* Participate in the development, implementation, and documentation of Personal Service Plans.
* Record daily activity and personal health information (PHI) of supported individuals on our case management database.
* Collaborate with individuals to maintain a clean and well-maintained home, including daily food preparation and light housekeeping duties.
* Possess a Community Support Worker Diploma or equivalent, with a minimum of 2 years of experience in community social services/health care.
* Hold current Emergency First Aid/CPR and Food Safe certificates, as well as previous behaviour management course/training.
* Demonstrate effective communication skills (verbal and written) with team members, professionals, families, and community members.
* Possess a restricted class 4 driver's license (for Full-Time positions).
* Obtain criminal record clearance for children and vulnerable adults, as well as proof of COVID-19 vaccination status.
