Key Responsibilities- Respect and Advocate: Ensure the supported persons' rights and choices are respected and advocate as needed.
- Effective Interactions: Interact effectively with people we support, fostering positive relationships and a sense of community.
- Empowerment and Independence: Empower the people we support to be full citizens of their community, promoting independence and self-determination.
- Personal Service Plans: Participate in the development, implementation, and documentation of Personal Service Plans, ensuring individualized care and support.
- Communication and Collaboration: Foster positive relationships with persons' family members, professionals, co-workers, and members of the community, promoting effective communication and collaboration.
- Record Keeping: Record the daily activity and personal health information (PHI) of the people we support on our case management database.
- Home Maintenance: Work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties.
- Education and Experience: Community Support Worker Diploma or equivalent, with a minimum of 2 years of experience in community social services/health care.
- Certifications: Current Emergency First Aid/CPR and Food Safe certificates, previous behaviour management course/training, and a restricted class 4 driver's licence (for Full-Time positions).
- Clearance and Vaccination: Criminal record clearance for children and vulnerable adults, and proof of COVID-19 vaccination status as per current Public Health Orders.
- Computer Literacy: Moderate computer literacy and ability to effectively communicate (verbal & written) with team members, professionals, families, and community members.
