Job Summary

We are seeking a highly skilled and experienced Home Support Team Lead to join our team at Spectrum Health Care. As a key member of our leadership team, you will be responsible for leading a team of Personal Support Workers (PSWs) in the field, providing ongoing training, coaching, and performance management.

Key Responsibilities

  • Lead a team of PSWs to provide high-quality personal care to patients in a home setting
  • Travel to patients' homes in community and facility settings to provide support and guidance
  • Work collaboratively with multidisciplinary teams in both community and facility settings to ensure seamless care delivery
  • Assess patient needs and develop individualized care plans to achieve optimal outcomes
  • Manage staff performance, conduct regular performance appraisals, and implement progressive disciplinary action as needed
  • Monitor and analyze patient and staff feedback to identify areas for improvement and implement changes
  • Ensure timely resolution of patient concerns and complaints
  • Represent Spectrum Health Care at local healthcare and business committees, as well as community events, to promote our services and build relationships
  • Participate in ongoing internal and external continuing education to stay up-to-date on best practices and industry developments

Requirements

  • Current registration with the Colleges of Nurses of Ontario in good standing
  • Minimum three years' experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting
  • Exceptional interpersonal skills and ability to handle difficult situations in a professional and objective manner
  • Excellent written and verbal communication skills
  • Strong knowledge of community nursing principles, practices, and methods, as well as program development, implementation, and evaluation
  • Valid driver's license and access to a reliable vehicle
  • Basic proficiency in Microsoft Word, Excel, and ability to learn proprietary software
