Job Summary

We are seeking a highly skilled and experienced Registered Practical Nurse (RPN) to fill the role of Home Support Supervisor at Spectrum Health Care Inc. This is a full-time position that requires the successful candidate to lead a team of Personal Support Workers (PSWs) in the field, providing ongoing training, coaching, and performance management.

Key Responsibilities
  • Team Leadership: Lead a team of Personal Support Workers to provide qualified personal care to patients in a home setting.
  • Travel and Community Outreach: Travel to patient's homes in community and facility settings to provide care and support.
  • Collaboration and Communication: Work collaboratively with multidisciplinary teams in both community and facility settings to ensure seamless care delivery.
  • Patient Assessment and Care Planning: Assess patient needs, formulate the plan of care, and assess progress towards outcomes.
  • Staff Management and Development: Manage staff, conduct performance appraisals, and implement progressive disciplinary action as needed.
  • Quality Improvement and Patient Feedback: Gather, record, and analyze patient and employee feedback to maintain an awareness of key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion rates.
  • Patient Concerns and Complaints: Ensure that all patient concerns and complaints are addressed and documented in a timely manner.
  • Professional Development and Community Engagement: Promote Spectrum Health Care through participation in local healthcare or business committees as well as community events, and participate in ongoing internal and external continuing education for personal development.
Requirements
  • Registration and Certification: Current registration with the Colleges of Nurses of Ontario in good standing.
  • Leadership Experience: Minimum three (3) years' experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting.
  • Interpersonal and Communication Skills: Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format.
  • Technical Skills: Ability to write reports and communicate effectively in writing and verbally, with excellent English verbal and written communication skills (Additional language skills considered an asset).
  • Knowledge and Skills: Solid knowledge of the principles, practices, and methods of community nursing and service delivery to patients, as well as program development, implementation, and evaluation.
  • Transportation and Technology: Valid driver's license and access to a reliable vehicle, with basic proficiency in Microsoft Word, Excel, and ability to learn proprietary software.
