Job Summary

We are seeking a highly skilled and experienced Home Support Team Lead to join our team at Spectrum Health Care Inc. As a key member of our leadership team, you will be responsible for leading a team of Personal Support Workers (PSWs) in the field, providing ongoing training, coaching, and performance management.

The ideal candidate will have a strong background in community nursing and service delivery, with a minimum of three years' experience in a supervisory role. You will be responsible for assessing patient needs, formulating care plans, and evaluating progress towards outcomes.

Key Responsibilities
  • Team Leadership: Lead a team of PSWs in the field, providing guidance, support, and performance management.
  • Patient Care: Assess patient needs, formulate care plans, and evaluate progress towards outcomes.
  • Staff Management: Manage staff, conduct performance appraisals, and implement progressive disciplinary action.
  • Communication: Communicate effectively with patients, families, and healthcare teams to ensure seamless care delivery.
  • Quality Improvement: Identify areas for quality improvement and implement changes to enhance patient care and staff performance.
Requirements
  • Education: Current registration with the Colleges of Nurses of Ontario in good standing.
  • Experience: Minimum three years' experience in a supervisory role, preferably in a community setting.
  • Skills: Exceptional interpersonal skills, ability to handle difficult situations, and excellent written and verbal communication skills.
  • Knowledge: Solid knowledge of community nursing and service delivery principles, practices, and methods.
  • Technology: Basic proficiency in Microsoft Word, Excel, and ability to learn proprietary software.
