Position Overview:

The Coordinator of Resident and Family Engagement plays a vital role in enhancing the lives of residents within our community. This position is essential for fostering connections and ensuring a positive experience for both residents and their families.

Key Responsibilities:

  • Facilitate Transitions: Oversee the resident's transition process, including move-ins, transfers, and departures, ensuring adherence to established standards and providing necessary support throughout.
  • Provide Counseling Services: Deliver social work counseling in collaboration with the interdisciplinary team, focusing on assessing and developing care plans that address the psychosocial needs of residents.
  • Develop Social Programs: Plan, implement, and evaluate tailored social work services and support groups to meet the unique needs of residents and their families.
  • Support Advanced Care Planning: Assist residents and families in discussions regarding advanced care planning and treatment options, including end-of-life arrangements.
  • Supervise Students: Mentor and oversee social work students during their placements, providing guidance and support.
  • Manage Feedback: Address and track feedback from residents and families to ensure timely and satisfactory resolutions.
  • Reporting and Analysis: Compile and present reports on feedback and outcomes to leadership and quality committees.

Qualifications:

  • Registered Social Worker in accordance with provincial regulations.
  • A minimum of three years of experience in healthcare, social services, or a related field.
  • Familiarity with the Long Term Care Act and Regulations is preferred.
  • Experience in customer service is advantageous.
  • Background in hospitality or sales is a plus.
  • Excellent verbal and written communication skills are essential.
  • Strong organizational and planning abilities, with the capacity to delegate tasks effectively.
