Resident & Family Experience Specialist

Glen Rouge Community is seeking a dedicated and compassionate Resident & Family Experience Coordinator III to join our team. This role plays a vital part in enhancing the lives of our residents by fostering a warm and supportive environment.
As a Resident & Family Experience Coordinator III, you will be responsible for:
- Guiding Residents Through Transitions: Lead the resident move-in, transfer, and move-out process, ensuring a smooth and positive experience. This includes assisting with applications, financial support access, tours, introductions to the community, and ongoing support for residents and their families as they adjust to long-term care.
- Providing Social Work Support: Deliver social work counseling services to residents, collaborating with the interdisciplinary team to assess needs and develop personalized care plans that address psychosocial and social well-being.
- Creating Engaging Programs: Plan, implement, and evaluate individual social work services and support groups tailored to the specific needs of residents and families. Utilize external resources to ensure comprehensive support for resident social needs.
- Facilitating End-of-Life Conversations: Support residents and families during advanced care planning and treatment plan discussions within your scope of practice, including sensitive conversations about funeral arrangements.
- Mentoring Future Social Workers: Supervise social work students during their placements, providing guidance and support as they gain valuable experience.
- Addressing Resident Feedback: Respond to resident feedback promptly and effectively, ensuring concerns are addressed in a timely manner and resolved to the satisfaction of residents and families. Provide regular reports and analysis on feedback received to leadership and quality committees.
Qualifications:
- Registered Social Worker (RSW) as per provincial legislation
- Minimum of three years of experience in healthcare, social services, or a related field
- Knowledge of the Long Term Care Act & Regulations is preferred
- Strong customer service skills are an asset
- Excellent oral and written communication skills
- Ability to organize, plan, and delegate responsibilities effectively
