Position Overview: The Resident & Family Experience Coordinator is an essential position within our organization, significantly influencing the lives of residents and fostering meaningful connections.

Key Responsibilities:

  • Facilitation of Transitions: Oversee the processes of resident admission, transfer, and discharge, ensuring adherence to established standards. This includes guiding families through application procedures, providing community tours, and assisting with the adjustment to long-term care living.
  • Social Work Services: Deliver counseling services as needed, collaborating with a multidisciplinary team to assess and create personalized care plans that address residents' psychosocial and social requirements, thereby enhancing their overall wellbeing and safety.
  • Program Development: Design, implement, and assess individualized social work services and support groups tailored to the specific needs of residents and their families. Engage with external resources as necessary to fulfill social needs.
  • Advanced Care Support: Assist residents and families in discussions regarding advanced care planning and treatment options, including end-of-life arrangements within the scope of social work.
  • Student Supervision: Recruit and mentor social work students during their placements, providing guidance and support.
  • Feedback Management: Manage responses and correspondence related to feedback received in the resident areas, ensuring timely resolution and satisfactory outcomes.
  • Reporting and Analysis: Compile and present reports on feedback to Leadership and Quality Committees for continuous improvement.

Qualifications:

  • Registered Social Worker in accordance with provincial regulations.
  • A minimum of three years of experience in healthcare, social services, or a related field.
  • Familiarity with the Long Term Care Act & Regulations is preferred.
  • Experience in customer service is advantageous.
  • Background in hospitality or sales is beneficial.
  • Excellent oral and written communication skills are essential.
  • Strong organizational, planning, and delegation abilities are required.
