Job Summary

We are seeking a Registered Nurse with experience in diabetes education to join our team as a Patient Educator. The successful candidate will provide education and support to patients with diabetes, working closely with our interdisciplinary team to ensure the best possible outcomes for our patients.

Key Responsibilities

  • Provide education and support to patients with diabetes, including teaching medication self-administration and insulin pump use
  • Assess patient needs and develop individualized education plans
  • Collaborate with the healthcare team to ensure comprehensive care for patients with diabetes
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

  • Current registration with the College of Nurses of Ontario
  • Basic Cardiac Life Support Certification
  • Experience working in a fast-paced ambulatory setting with patients with complex medical needs
  • Excellent interpersonal, communication, and decision-making skills
  • Excellent organizational and time management skills
