Job Summary
The University Health Network is seeking a highly skilled Patient Educator Diabetes Specialist to join our Diabetes/Endocrine Program team. As a key member of our multi-disciplinary team, you will play a vital role in providing patient education and support to individuals with diabetes.
Key Responsibilities
- Develop and implement patient education programs to promote self-management of diabetes
- Provide individualized education and support to patients with diabetes, including teaching medication self-administration and glucose monitoring
- Collaborate with healthcare providers to ensure comprehensive care and support for patients with diabetes
- Conduct health assessments and develop individualized care plans to meet the unique needs of patients with diabetes
- Stay up-to-date with the latest research and best practices in diabetes care and education
Requirements
- Current registration with the College of Nurses of Ontario
- Basic Cardiac Life Support Certification
- Excellent interpersonal, communication, and decision-making skills
- Excellent organizational and time management skills
- Experience working in a fast-paced ambulatory setting with patients with complex medical needs
- Health assessment skills
- Experience with patients with diabetes, lipid disorders, thyroid disease, osteoporosis, and disorders of other endocrine glands
