Job Summary
The University Health Network is seeking a highly skilled and compassionate Registered Nurse to join our Diabetes/Endocrine Program as a Patient Educator. In this role, you will work collaboratively with a multi-disciplinary team to provide patient education and support to individuals with diabetes, ensuring they receive the highest quality care and achieve optimal health outcomes.
Key Responsibilities
- Provide patient education on diabetes management, including medication self-administration, insulin pump therapy, and glucose monitoring
- Develop and implement individualized education plans to meet the unique needs of patients
- Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes
- Conduct health assessments and provide health promotion and disease prevention education
- Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
- Current registration with the College of Nurses of Ontario
- Basic Cardiac Life Support Certification
- Excellent interpersonal, communication, and decision-making skills
- Ability to work effectively in a fast-paced ambulatory setting
- Experience working with patients with complex medical needs
- Health assessment skills and knowledge of diabetes management
