Job Summary

The University Health Network is seeking a highly skilled and compassionate Registered Nurse to join our Diabetes/Endocrine Program as a Patient Educator. In this role, you will work collaboratively with a multi-disciplinary team to provide patient education and support to individuals with diabetes, ensuring they receive the highest quality care and achieve optimal health outcomes.

Key Responsibilities

  • Provide patient education on diabetes management, including medication self-administration, insulin pump therapy, and glucose monitoring
  • Develop and implement individualized education plans to meet the unique needs of patients
  • Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes
  • Conduct health assessments and provide health promotion and disease prevention education
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

  • Current registration with the College of Nurses of Ontario
  • Basic Cardiac Life Support Certification
  • Excellent interpersonal, communication, and decision-making skills
  • Ability to work effectively in a fast-paced ambulatory setting
  • Experience working with patients with complex medical needs
  • Health assessment skills and knowledge of diabetes management
