Job Summary

We are seeking a highly skilled Registered Nurse to join our Diabetes/Endocrine Program as a Patient Educator. In this role, you will work closely with patients to provide education and support in managing their diabetes. You will also be responsible for developing and implementing patient education programs, as well as collaborating with other healthcare professionals to ensure comprehensive care.

Key Responsibilities

  • Provide patient education on diabetes management, including medication self-administration and glucose monitoring
  • Develop and implement patient education programs to promote healthy behaviors and disease management
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for patients
  • Conduct health assessments and develop individualized care plans for patients
  • Stay up-to-date with the latest research and best practices in diabetes care and education

Requirements

  • Current registration with the College of Nurses of Ontario
  • Basic Cardiac Life Support Certification
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Experience working with patients with complex medical needs
