Director of Care

At Extendicare Canada Inc., we are seeking a highly skilled and experienced Director of Care to join our team. As a key member of our medical team, you will be responsible for providing care to residents in accordance with the standards of practice and offering leadership and guidance to the personal care staff.
Key Responsibilities:- Implementation and evaluation of nursing department quality improvement initiatives, risk management systems, data collection and analysis, report writing, and procedural updates.
- Leading the coordination and implementation of best practice and risk management initiatives related to resident care, including falls, restraint use, wound management, and Infection Control.
- Effectively managing the utilization of nursing supplies.
- Responsible for the High Intensity Needs program.
- Maintaining a system of care assessment, planning, implementation, and evaluation at the resident and unit program level that maximizes staff resources and contributes to the Extendicare interdisciplinary team approach to resident care.
- Effectively supporting the RAI-MDS program.
- Maintaining an active presence in the Resident Home Areas, mentoring staff while overseeing the provision of quality resident care.
- Initiating investigative and corrective actions related to occupational health and safety; developing and monitoring modified work programs for nursing personnel.
- Working collaboratively with the Director of Care on special projects, reviews, and other duties as assigned.
- Identifying areas of improvement for staff development and working with the DOC to ensure overall effectiveness of the nursing department.
- Advocating for ethical and effective practice in all areas of the organization.
- Completing Performance Appraisals with the inputs of other members.
- Focusing on Resident Centered Care Needs as required by MOH LTC Home Legislation and Extendicare Policies.
- Actively participating in the palliative care program, goals of care, and advanced care planning.
- Being responsible for the nursing department.
- Successful candidates will be required to work one weekend per month.
- Current Certification in Infection Control (CIC) is preferred, or, willingness to complete by April 2025.
- Registered Nurse registered with College of Nurses of Ontario; BScN an asset.
- Minimum of three years experience in long term care; previous management experience an asset.
- Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education, and quality improvement.
- Knowledge of RAI MDS is required.
