Rebound Child & Youth Services is a not-for-profit, incorporated, charitable organization, providing high-quality, community-based programs and services to children, youth, and families in Northumberland County since 1997.
Rebound believes that the most effective services are those that are provided in a pro-active and responsive manner; prioritizing community needs identified by families themselves. As part of that commitment, Rebound has taken the lead in the opening of a Youth Wellness Hub in Northumberland County as part of the expansion of Youth Wellness Hubs Ontario’s (YWHO) provincial initiative alongside our Collaborative Partnership Team.
**ABOUT YOUTH WELLNESS HUBS ONTARIO (YWHO)**
YWHO supports local service providers to work together to provide young people in Ontario aged 12 to 25 with low-barrier access to a full range of services including but not limited to primary care, mental health and substance use counselling, service navigation, peer support and community and social supports all through youth-friendly access points.
With an anticipated opening in the summer of 2025, the **Youth Wellness Hub Northumberland** will offer integrated and holistic services that focus on early intervention and collaboration among various service providers in order to address gaps in the healthcare system for young people. The main goal is to improve experiences and outcomes for youth by increasing access to rapid, low-barrier services, reducing wait times for vital primary care/mental health services and reducing transportation barriers. Until the physical space is opened, staff will begin offering virtual services to community members in March of 2025.
The Hub would be the first and only Youth Wellness Hub available between Scarborough, Haliburton & Kingston ON, significantly increasing accessibility of services across the region.
**ABOUT THE ROLE**
The **Peer Support Worker** will work as a member of a diverse interdisciplinary Youth Wellness Team to support youth experiencing mental health, substance use or other challenges. Peer Support involves emotional and practical support between people who share a common experience. Peers help to build equal and empathetic relationships, encouraging young people to make decisions based on their knowledge and expertise about their own needs.
As a Peer Support Worker, you will utilize your lived/living experience of mental health and/or substance use to support youth accessing the Hub to determine their unique goals and service needs. In this role, you will provide education about navigating mental health and substance use challenges as well as where to find resources in the community to address those concerns. As part of the Youth Wellness Team, Peers contribute to program development and outreach activities related to YWHO’s Clinical Services, Skills & Well-being Activities and Community Support Services. Peers are also responsible for co-creating a welcoming space for youth who access Hub services, attending drop-in activities, as well as one-on-one mentorship.
**MAIN RESPONSIBILITIES**
- Engage youth and create a welcoming environment for youth accessing Hub services.
- Connect with clients on an individual basis and also as a group facilitator.
- Support youth with intake process/My Wellness Passport ( Measurement-based care Platform).
- Facilitate social and recreational activities, both within the Hub and (when relevant) community-based outings (e.g. gyms, community centres, land-based programming).
- Provide individual mentorship and coaching to youth to navigate mental health system.
- Assist in the planning, outreach and co-facilitation of skills activities and drop-in programming.
- Complete documentation of client interactions and timely completion of data entry, case notes and other electronic records as required.
- Attend team meetings as required and contribute to integrated care planning for youth.
- Participate in community network meetings and support outreach.
- Provide virtual-based (online, phone) and in-person service.
- Participate in YWHO Provincial Community of Practice for Peer Support Worker role.
- Work from a harm reduction and culturally responsive approach.
- Other duties as assigned.
**QUALIFICATIONS/EXPERIENCE REQUIRED**
- Lived/living experience as a young person with mental health and/or substance use challenges, which may include experience as a recipient of mental health and/or substance use services, is required.
