Your Opportunity:

As a skilled Oncology Registered Nurse, you will play a pivotal role as the Head and Neck Cancer Patient Navigator (H&N CPN) within Cancer Care Alberta (CCA). Your primary focus will be to enhance the care continuum for patients diagnosed with head and neck cancers at the Cross Cancer Institute (CCI). As part of our provincial navigation team, you will ensure seamless transitions throughout the treatment journey, from surgical interventions to outpatient therapies, including radiation and systemic treatments, rehabilitation, follow-up care, and into survivorship or palliative care.

In collaboration with multidisciplinary healthcare teams, you will conduct thorough symptom assessments and management, addressing patients' concerns, providing essential resources regarding their condition and treatment options, and facilitating referrals to additional services as necessary. Your role will involve delivering personalized interventions and support to meet the diverse physical, psychological, social, emotional, spiritual, and practical needs of patients through both in-person and virtual consultations.

Key Responsibilities:

  • Engage with key stakeholders, including supportive care services, community providers, surgical teams, and palliative care units.
  • Educate patients and families on understanding test results, navigating their care pathways, and identifying potential care gaps.
  • Promote self-management and informed decision-making, while supporting patients in achieving their care goals.
  • Facilitate referrals from community healthcare providers and coordinate with various clinics and triage coordinators.

Description:

As a Registered Nurse (RN), you will deliver a comprehensive range of nursing services to individuals, families, and communities, ensuring their safety and well-being at all times. Your responsibilities will include employing nursing processes that encompass critical thinking, problem-solving, and decision-making, alongside teaching, counseling, and advocating for patients and their families.

You will assume a leadership role in fostering an integrated and holistic approach to patient care, health promotion, and maintenance. Your commitment to providing safe, high-quality, patient-centered care will reflect the core values and vision of Alberta Health Services.

Required Qualifications:

  • Completion of an accredited nursing education program.
  • Active registration or eligibility for a practice permit with the College of Registered Nurses of Alberta (CRNA).
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.

Additional Required Qualifications:

  • Completion of the CCA-AHS Cancer Patient Navigation course within three months of hire.
  • Three to five years of recent clinical experience in oncology.
  • Ability to thrive in a complex environment, managing multiple tasks while supporting patients experiencing emotional distress.

Preferred Qualifications:

  • CNA Oncology Certification (CONc) or Certification in Hospice Palliative Care (CHPCC) is preferred.
  • Experience with head and neck cancer patients is highly valued.
  • Background in community health nursing, palliative care, home care, or ambulatory care is considered an asset.
  • Familiarity with Connect Care training in Oncology and Ambulatory Care is advantageous.
