As a Social Worker (Full-Time), you will be tasked with delivering essential social work services to clients, aiming to optimize their psychosocial well-being, avert unnecessary decline, and foster independence.

Whether you operate from our headquarters, engage with the community, or work within a healthcare setting, you will be pivotal in offering integrated, accessible, and patient-focused care. You will be part of a collaborative team comprising over 9,000 regulated health professionals.

Key Responsibilities

Respond to referrals from Care Coordinators to effectively provide social work services to clients. Evaluate clients to identify and assess their psychosocial needs and therapeutic objectives. Implement treatment programs, counseling techniques, and emotional support for clients. Monitor client progress and modify treatment plans as necessary to ensure therapy goals are achieved. Educate and guide clients, their families, caregivers, and/or substitute decision-makers regarding challenges and strategies for ongoing improvement. Maintain accurate documentation of client assessments, goals, treatment plans, and interventions. Connect clients with additional in-house services or external organizations as required. Balance client needs and preferences with available resources, ensuring their values are honored.

Required Qualifications

Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW). Relevant experience in social work within community or healthcare settings. In-depth knowledge of social work practices, interventions, and professional competencies. Familiarity with community resources, funding bodies, treatment facilities, and social service networks. Understanding of the roles of various healthcare professionals. Current knowledge of social work theories, methodologies, and procedures. Ability to effectively prioritize responsibilities and manage caseloads efficiently. Strong interpersonal and communication skills. Proficient listening, observation, and facilitation abilities. Excellent mediation, negotiation, and conflict resolution skills for emergency situations. Proficient in computer software (email, internet) and Microsoft Office applications (Word, Outlook). Valid driver's license and access to a reliable vehicle are required. Advanced proficiency in English, both oral and written, is essential. Compliance with the mandatory COVID-19 vaccination policy is required for employment.

Preferred Qualifications

Experience working with diverse populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics. Case management experience or recent relevant community experience. Proficiency in French or another second language is an asset.
