Position Overview

As a Care Coordinator, you will evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Your primary objective will be to assist patients in achieving better health outcomes at home, while you enjoy our comprehensive support for professional development, personal wellness, and work-life harmony.

Key Responsibilities

Collaborate with patients and their families to assess care requirements, determine service eligibility, and create personalized care and service plans. Connect patients with appropriate service providers. Oversee and manage the implementation of care plans. Build supportive relationships with patients and their families. Balance patient preferences and needs with available resources, ensuring their values are honored.

Required Qualifications

Active membership in the relevant regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers. Recent experience in community health or a related field. Familiarity with the healthcare delivery system and community resources. Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities. Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic multidisciplinary environment. Proven ability to accurately complete necessary documentation, reports, and forms. A valid driver's license and access to a reliable vehicle. Proficiency in a Windows operating environment. Advanced oral and written proficiency in English and French is essential. Compliance with a mandatory COVID-19 vaccination policy is required.

Preferred Qualifications

Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics. Background in case management or relevant community experience. Ability to communicate in French or another second language.
