Position Overview

As a Care Coordinator, you will evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, families, and healthcare providers.

Whether your work environment is an office, community setting, or healthcare facility, you will be instrumental in delivering integrated, patient-focused care, supported by a collaborative team of over 9,000 healthcare professionals.

Key Responsibilities:

Collaborate with patients and their families to assess care needs, determine service eligibility, and create personalized care plans Connect patients with appropriate service providers Oversee and evaluate the implementation of care plans Build supportive relationships with patients and their families Balance patient preferences with available resources, ensuring respect for their values and choices

Required Qualifications:

Active membership with the relevant regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers Recent experience in community health or a related sector Familiarity with the healthcare delivery system and community services Strong interpersonal, communication, assessment, problem-solving, and decision-making abilities Effective time management, prioritization, and organizational skills, with the capacity to work independently and collaboratively in a dynamic multidisciplinary environment Proven ability to accurately complete necessary documentation and reports A valid driver's license and access to reliable transportation Proficient in a Windows operating environment Advanced proficiency in both oral and written English and French is essential Compliance with a mandatory COVID-19 vaccination policy is required.

Preferred Qualifications:

Experience working with diverse patient populations, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics Background in case management or related community experience Ability to communicate in French or another second language
