Job Summary

We are seeking an experienced Occupational Therapist to join our team at Home and Community Care Support Services. As an Occupational Therapist, you will play a vital role in providing high-quality care to our patients, maximizing their function, independence, and safety in their respective environments.

Key Responsibilities
  • Assess the factors influencing the client's ability to function in their environment, using a variety of standardized and non-standardized assessment tools.
  • Determine patient-centered therapy goals and provides treatment programs to increase occupational performance, independence, and safety.
  • Share knowledge and information with other healthcare professionals and agencies to work collaboratively to deliver effective and efficient patient care.
  • Build therapeutic relationships by ensuring patients' values and preferences are respected and balancing patient needs and choices with available resources.
  • Document assessment findings, goals, and intervention plans in accordance with HCCSS policies and COTO standards for record keeping.
Requirements
  • Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario.
  • In-depth knowledge of occupational therapy services and other community resources.
  • Good understanding of the roles of healthcare professionals.
  • Relevant occupational therapy experience in a community/healthcare environment.
  • Comprehensive and up-to-date knowledge of occupational therapy tools, processes, equipment, and assistive technology.
  • Knowledge of funding agencies related to occupational therapy recommendations.
  • Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, health, and safety.
  • Comprehensive knowledge of the standards of practice and professional guidelines set forth by COTO.
  • Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload.
  • Strong interpersonal and collaboration skills to work with diverse patient groups, case managers, family members/caregivers/Substitute Decision Makers, healthcare professionals, community organizations, and service providers.
  • Effective listening, observation, and facilitation skills.
  • Ability to communicate information effectively through a variety of means, including reports, letters, meetings, and presentations.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel).
  • Must have a valid driver's license and access to a vehicle.
  • Advanced written and oral proficiency in English and French is essential.
Preferred Qualifications
  • Experience working with diverse patient groups, case managers, family members, caregivers, SDMs, healthcare professionals, community organizations, and/or service providers in Community Support Services.
  • Case management experience in a healthcare setting and/or Community Support Services.
  • Ability to speak French or another second language.
