As a Social Worker (Full-Time), you will be entrusted with delivering essential social work services to patients, aiming to enhance their psychosocial well-being, avert unnecessary decline, and foster their independence.

Whether your role is based in our office, within the community, or at a healthcare facility, you will be pivotal in providing integrated, accessible, and patient-focused care. You will collaborate with a diverse team of over 9,000 regulated health care professionals, ensuring a holistic approach to patient care.

Key Responsibilities

  • Respond to referrals from Care Coordinators to effectively deliver social work services to patients.
  • Conduct assessments to identify patients' psychosocial needs and establish therapeutic objectives.
  • Develop and implement treatment programs, counseling strategies, and provide emotional and mental health support.
  • Monitor patient progress and modify treatment plans as necessary to meet therapy goals.
  • Educate and guide patients, their families, caregivers, and/or Substitute Decision Makers (SDMs) on relevant issues and strategies for ongoing improvement.
  • Maintain accurate documentation of patient assessments, goals, treatment plans, and interventions.
  • Connect patients with additional in-house services or external organizations as required.
  • Balance patient preferences with available resources, ensuring their values are honored.

Required Qualifications

  • Registered Social Worker in good standing with the relevant regulatory body.
  • Experience in social work within community or healthcare settings.
  • In-depth knowledge of social work practices, interventions, and professional skills.
  • Familiarity with community resources, funding agencies, treatment centers, and social service networks.
  • Understanding of the roles of various healthcare professionals.
  • Current knowledge of social work theories, practices, and protocols.
  • Ability to manage caseloads efficiently while prioritizing professional responsibilities.
  • Strong interpersonal and communication skills.
  • Proficient listening, observation, and facilitation abilities.
  • Effective mediation, negotiation, and conflict resolution skills for emergency situations.
  • Competence in using computer software, including Microsoft Office applications.
  • Valid driver's license and access to reliable transportation.
  • Proficiency in English, both spoken and written, is essential.
  • Compliance with the organization's vaccination policy is mandatory.

Preferred Qualifications

  • Experience working with diverse populations, including multicultural, homeless, palliative, and mental health groups.
  • Background in case management or related community experience.
  • Ability to communicate in French or another second language.
