As a Social Worker (Full-Time), you will provide essential social work services to clients, aiming to enhance their psychosocial well-being, prevent unnecessary decline, and foster independence.

Key Responsibilities

Respond to referrals from Care Coordinators to effectively provide social work services to clients. Evaluate clients to identify their psychosocial needs and establish therapeutic treatment objectives. Develop treatment programs, counseling techniques, and emotional support for clients. Monitor client progress and modify treatment plans as necessary to ensure therapeutic goals are achieved. Educate and support clients, their families, caregivers, and/or substitute decision-makers regarding challenges and strategies for ongoing improvement. Maintain accurate documentation of client assessments, goals, treatment plans, and interventions. Connect clients with additional in-house services or external organizations as required. Balance client needs and preferences with available resources, ensuring their values are respected.

Required Qualifications

Registered Social Worker in good standing with the relevant regulatory body. Relevant experience in a community or healthcare setting. Comprehensive understanding of social work interventions and professional practices. Familiarity with community resources, funding agencies, treatment facilities, and social service networks. Good insight into the roles of other healthcare professionals. Current knowledge of social work theories, practices, and procedures. Ability to prioritize responsibilities and manage caseloads efficiently. Strong interpersonal and communication skills. Effective listening, observation, and facilitation abilities. Proficient mediation, negotiation, and conflict resolution skills for emergency situations. Proficient in computer software (email, internet) and Microsoft Office applications (Word, Outlook). Valid driver's license and access to reliable transportation. Advanced proficiency in English, both oral and written, is essential. Compliance with the organization's vaccination policy is mandatory.

Preferred Qualifications

Experience working with diverse populations, including multicultural, homeless, palliative, mental health, and pediatric groups. Case management experience or relevant community involvement. Proficiency in French or another second language.
