Position Title: Advanced Practice Nurse - Cardiac Care Program

Position Overview:

  • Report directly to the Director of the Cardiac Care Program
  • Serve as a vital member of the Cardiac Care team

Key Responsibilities:

  • Deliver high-quality, evidence-informed care in partnership with patients and their families
  • Apply advanced clinical expertise to enhance health outcomes and care quality
  • Collaborate with interdisciplinary teams to improve care delivery across diverse environments
  • Facilitate professional growth and mentorship within the Cardiac Care Program
  • Design and execute policies and clinical pathways aimed at quality enhancement
  • Provide cross-coverage within the cardiac program as necessary

Required Qualifications:

  • Master's degree in Nursing/Nurse Practitioner Stream or Master's in Nursing with Acute Care NP Certificate
  • A minimum of 1 year of experience as an advanced practice nurse in Cardiovascular Surgery
  • At least 3 years of relevant clinical specialty experience as a Registered Nurse
  • Current membership with RNAO and NP Association of Ontario is preferred
  • Proven ability to work independently and collaboratively in an inter-professional environment
  • Experience in program development, quality improvement initiatives, and clinical research
  • Strong organizational, communication, leadership, and problem-solving capabilities
  • Dedication to customer service, teamwork, and ongoing professional development
