Position Overview

The Licensed Practical Nurse (LPN) exemplifies clinical proficiency and adheres to the core competencies outlined by the College of Nurses of Ontario (CNO), alongside the policies and procedures of Southlake Regional Health Centre. The LPN collaborates effectively as a vital member of the healthcare team, delivering direct patient care grounded in the three-factor framework established by the CNO standard, which encompasses the Client, Nurse, and Environment. These elements are crucial in guiding decision-making regarding care assignments and the necessity for interdisciplinary collaboration.

Key Responsibilities

• Deliver patient and family-centered care within the LPN scope of practice, utilizing the three-factor framework from the College of Nurses of Ontario.
• Operate in alignment with established hospital protocols and procedures, including emergency response codes.
• Ensure practice adheres to entry-level competencies, standards, guidelines, and relevant legislation.
• Communicate effectively with patients, families, and all healthcare team members.
• Collaborate with Registered Nurses (RNs) to provide comprehensive care for patients with complex health conditions.
• Work alongside other interprofessional healthcare team members.
• Offer consultation and clinical support to colleagues as required.
• Contribute to the ongoing assessment and documentation of each patient's health status.
• Assist in developing and implementing the Plan of Care, evaluating its effectiveness regularly.
• Participate in the orientation and mentorship of new staff and students within clinical units.
• Serve as a role model, promoting the integration of safe, best practices into daily operations to ensure exemplary care delivery.
• Perform additional duties as assigned by the Manager or delegate.

Qualifications & Skills

• Recent experience in acute general medicine nursing is preferred.
• Familiarity with the initiation and maintenance of IV therapy is advantageous.
• Experience in health education is preferred.
• Ability to work 12-hour shifts, including nights and weekends.
• Proficient in accessing, maintaining, and blood sampling from central venous access devices (CVAD).
• Knowledge of complex and geriatric medical populations.
• Demonstrated expertise in skin assessment and wound care.
• Ability to prioritize and organize tasks based on patient needs.
• Capability to function within the full scope of nursing practice as per the College of Nurses of Ontario.
