L’équipe de santé familiale académique Montfort est un groupe dynamique, formé de médecins et autres professionnels de la santé. Notre vision est de « maintenir et améliorer la santé de nos patients en offrant les soins compréhensifs et intégrés en français et en anglais, et aussi d’éduquer les médecins francophones de l’avenir ».

Nous sommes à la recherche d’une personne autonome et motivée afin d’occuper un poste d’administrateur de programmes à temps plein.

**RÉSUMÉ DES FONCTIONS**:
L’administrateur de programmes appui l’Équipe de santé familiale académique Montfort (ESFAM) en développant et en assurant le maintient de ses programmes de gestion de maladies chroniques et de promotion de la santé. Il travaille de près avec les responsables de programmes, ainsi qu’avec la direction générale, dans le but de promouvoir la santé de nos patients.

**Saisie de données **:Le titulaire assure la saisie de données dans le but de préparer des évaluations de programmes et de divers rapports.

**Développement et mise en œuvre **:L’administrateur de programmes travail à développer, mettre en oeuvre et assurer le bon déroulement des programmes de l’ESFAM.

**Coordination des patients **:Le titulaire agit comme agent de liaison entre l’équipe de soutien administratif, les fournisseurs interdisciplinaires en services de la santé (FISS) et la direction générale dans le but d’assurer un flux de patients adéquat pour participation aux programmes.

**Évaluation **:L’administrateur de programmes assure que les programmes sont assujettis à une évaluation avant de commencer une vague ultérieure afin de permettre la mesure du rendement et l’apport d’améliorations.

**COMPÉTENCES EXIGÉES**
- Possède un diplôme universitaire en sciences de la santé, santé publique ou sciences infirmières;
- **Capacité de communiquer avec aisance, verbalement et par écrit, tant en français qu’en anglais ;**:

- Compréhension des approches fondées sur des équipes de soins primaires et multidisciplinaires pour la prestation de soins de santé;
- Connaissances informatiques raisonnables;
- Deux (2) à trois (3) années d’expérience antérieure dans un milieu lié à la santé ou aux services sociaux en implémentation de projets ou de programmes en santé;
- Expérience en élaboration et en production de rapports de rendement conviviaux;
- Excellentes aptitudes pour la gestion de projet;
- Compétences analytiques démontrées;
- Fais preuve de collaboration.

**ATOUTS**:

- Possède une maitrise en administration de la santé, une maitrise en santé publique, ou autre éducation supérieure équivalente,
- Connaissance et expérience des dossiers médicaux électroniques (DME);
- Travail antérieur dans le domaine de planification des services de santé primaire.

**SALAIRE**:
Le titulaire sera rémunéré selon une échelle salariale annuelle à cinq (5) niveaux d’entre 51 300$ à 60 400$

The Montfort Academic Family Health Team is a dynamic group of physicians, nurses and other health professionals whose vision is to “maintain and improve the health of our patients by providing comprehensive and integrated care in both French and English, and to educate the francophone primary care physicians of the future”.

The Montfort Academic Family Health Team is looking for an energetic and self-motivated person to fulfill the full-time position of Programs Administrator within its interdisciplinary health team.

***:
The Programs Administrator supports the Montfort Academic Family Health Team in developing and maintaining its chronic disease management and health promotion programs. The incumbent works closely with the program leads, in addition to senior management, in promoting the team’s services to its patients.

**Data collection**:The incumbent collects program data in order to facilitate planning and evaluation of programs.

**Development and implementation**: The Programs Administrator works with the interdisciplinary team to create and implement new programs.

**Patient coordination**: The Programs Administrator acts as a liaison between interdisciplinary healthcare providers, physicians and the administrative team in order to ensure that all programs are marketed and that sufficient cohorts of patients are included as participants.

**Evaluation**: The incumbent is responsible for evaluating programs at their end of life cycle in efforts to foster improvements and in order to report on performance indicators.
