About the Opportunity

The Care Coordinator is responsible for assessing the needs of clients referred to Continuing Care and developing personalized care plans to support their health and well-being. This role involves working with interprofessional teams to facilitate hospital discharge and ensure a smooth transition to community-based care.

Care Coordinators work in a variety of settings, including community, hospital, and long-term care facilities. They are responsible for authorizing services and making referrals to other resources to support clients in achieving their goals. This role requires strong assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills.

The ideal candidate will have a Bachelor's degree in a relevant field, such as nursing, social work, or occupational therapy, and a minimum of three years' experience in a community or long-term care setting. They will also have knowledge of community agencies and resources, as well as experience in dedicated case management or discharge planning.

  • Valid driver's license and access to a reliable vehicle

Hours
  • Long Assignment Full-time / 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$41.06 - $46.75 Hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
