About the Opportunity

We are seeking a skilled and experienced Continuing Care Coordinator to join our team at Nova Scotia Health. As a key member of our care coordination team, you will provide leadership, innovation, and support with assessment, authorization, service planning, and collaborative case management aligned with provincial policies for continuing care programs and services.

The successful candidate will work closely with continuing care staff and managers, contracted home support, nursing and long-term care service providers, NSH clinical programs and team members, primary healthcare providers, and community agencies and organizations to promote person-centered care.

Key responsibilities will include fostering healthy team processes, facilitating collaboration between program areas and their networks, assisting in program planning and evaluation, participating in the orientation and mentorship of new team members, supporting a quality approach to service delivery, and assisting with the development of consistent policies related to continuing care programs and services.

This role encompasses a portion of a pre-existing Care Coordinator position, requiring the successful candidate to work as part of a team with clients, peers, and other healthcare professionals while exercising team leadership functions.

About You

We are looking for a candidate with a strong background in healthcare, preferably with a Bachelor's degree in Social Work, Occupational Therapy, Physiotherapy, Nutritional Studies, or Nursing. A minimum of five years of related experience and current registration with the applicable provincial licensing body are also required.

The ideal candidate will possess excellent leadership capabilities, the ability to conduct comprehensive multi-dimensional assessments, and experience developing and evaluating complex care plans. Strong communication and interpersonal skills, as well as proficiency in computer applications, are also essential.

Hours and Compensation

This is a long-assignment, full-time position with a 70-hour biweekly schedule. The successful candidate will be eligible for our benefits package, including health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.

The hourly rate for this position is $42.56-$48.27, depending on experience. We are committed to providing a competitive compensation package and a supportive work environment that fosters growth and development.
