As a Continuing Care Coordinator, you will be responsible for:

  • Assessing clients' needs and developing personalized care plans
  • Coordinating services and resources to support clients in the community
  • Working with interprofessional teams to facilitate hospital discharge and return to the community
  • Providing education and support to clients and their families

We offer a competitive salary range of $41.06 - $46.75 per hour, as well as a comprehensive benefits package and opportunities for professional growth and development.
