About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating care for clients in the community.

You will work closely with clients, families, caregivers, and healthcare teams to identify goals, unmet needs, and resource requirements. Based on your assessment, you will assist clients and their care partners to formulate a plan to meet identified goals.

As a Continuing Care Coordinator, you will authorize services and make referrals/linkages to other professional and volunteer resources to support community-dwelling clients. You will work in the community, hospital, or long-term care facilities, and be fiscally responsible for the care, services, and equipment you authorize within available Department of Seniors and Long Term Care policies and programs.

This is a unique opportunity to work as a community-based Continuing Care Coordinator as part of a Primary healthcare team in the community, with an increased emphasis on interprofessional collaboration with the larger home health team.

About You

We are looking for a highly skilled and experienced professional with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. You must have current and ongoing registration (or eligibility for registration) with the applicable Nova Scotia Professional Regulatory body, and a minimum of three years' experience with the professional regulatory body.

Preference will be given to candidates with more than five years' experience or those who have experience working in the community. You should have demonstrated knowledge and/or experience in a community or long-term care setting, and be competent with computer use, keyboarding, and able to conduct assessments using a laptop.

You will have excellent assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. You must be able to work independently and co-operatively in a busy multidisciplinary situation, and be able to multi-task, prioritize workload, and manage time efficiently.

Hours

This is a permanent full-time position, with 70 hours bi-weekly. Shifts may include days, evenings, weekends, and holidays.

Compensation and Incentives

The successful candidate will be offered a competitive salary of $41.06 - $46.75 hourly, and may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
