About the Opportunity

We are seeking a skilled and compassionate Continuing Care Coordinator to join our team at Nova Scotia Health Authority. As a key member of our healthcare team, you will play a vital role in assessing, planning, and coordinating care for clients in the community.

The ideal candidate will have a strong background in nursing, social work, or a related field, with a minimum of three years of experience in a healthcare setting. You will possess excellent assessment, interviewing, and negotiation skills, as well as the ability to work independently and collaboratively in a fast-paced environment.

As a Continuing Care Coordinator, you will be responsible for:

  • Assessing clients' needs and developing personalized care plans
  • Coordinating services and resources to support clients in the community
  • Working with interprofessional teams to facilitate hospital discharge and return to the community
  • Providing education and support to clients and their families

We offer a competitive salary range of $41.06 - $46.75 per hour, as well as a comprehensive benefits package and opportunities for professional growth and development.

About You

We are looking for a candidate who:

  • Has a Bachelor's degree in nursing, social work, or a related field

  • Has a valid driver's license and access to a reliable vehicle
