About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. They work with interprofessional teams to facilitate hospital discharge and support community-dwelling clients. The Care Coordinator maintains ongoing communication with clients, family, caregivers, and healthcare teams within the client's circle of care.

They are responsible for reassessments at routine intervals and skilled care management. The philosophy of the Care Coordinators is grounded in 'Home First': maximizing available services and resources to support clients and families in the home/community.

About You

We are seeking a qualified professional with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body and a minimum of three years' experience in a community or long-term care setting.

They will have demonstrated knowledge and/or experience in dedicated case management or discharge planning, as well as excellent assessment, interviewing, and negotiation skills. The ability to work independently and co-operatively in a busy multidisciplinary situation is essential.

Hours and Compensation

This is a permanent, full-time position with a 70-hour bi-weekly schedule. Shifts may include days, evenings, weekends, and holidays. The successful candidate will be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
